Chemical Inventory
Lists
Each
department must fill out chemical inventory lists documenting all chemicals and chemical
products in their area. These lists are turned in to EHS where they are kept on file.
These lists provide vital information in case of emergency.
Employee Exposure Report
Forms
If
you are exposed to potentially hazardous substances that could result in acute (short
term) or chronic (long term) health effects, you should fill out an Employee Exposure
Report Form.
These
reports may be filled out to document exposures to any hazardous substances including
chemicals, asbestos, blood borne pathogens, or others.
If
your supervisor does not have a copy of the form, they are available upon request from
Environmental Health and Safety. Once completed, the form should be returned to EHS where it
will remain on file for a period of 40 years after you have ceased employment at OSU.
The
reason the report will remain on file for so long is simple: if you are exposed to a
hazardous substance such as asbestos, it may take 20-40 years to develop an asbestos
related disease. This report is your documentation that you had work-related exposure to
asbestos. It is a protective measure to ensure that you may be able to receive Workers'
Compensation to help pay your medical bills for a work-related illness.
It
should be understood that filling out this form is not an admission of liability on the
part of OSU or the employee. These forms will not be used against the employee in any
way. They are provided for your protection.
